At this time we are only accepting patients on a very limited basis. Please contact our office at (805) 639-9332 to inquire about our current availability.
If you have been accepted as a new patient we will begin the registration process over the phone. Once we have registered you in our system you will receive a text or email with information about our patient portal. You will also receive a separate email with a link for our demographic forms or you can click the link above. The demographic forms can be completed on your device and then printed off to be signed. You can return them to us by mail, email or bring them to your first appointment.
A few days before your first appointment you will receive a reminder text with a link to pre-check-in. During this check-in process you will be asked to complete medical history forms. Completing these prior to your appointment will give us time to have your chart updated for the doctor. We appreciate your help in getting your account up-to-date in our system.
